A practical procurement checklist for administrators who buy wheelchairs, ICU monitors, lab analyzers, and nebulizers. Seven steps to get the right manuals, error code lists, training, and service support before you sign.

Who This Checklist Is For

If you've ever signed a quote for medical equipment and then spent three weeks chasing a manual, an error-code guide, or a training session, this checklist is for you. It's written from the purchasing side, not the clinical side. I manage buying for an 85-person rehab and diagnostics center. My orders include electric wheelchairs, ICU monitors, hematology analyzers, and patient-use items like nebulizers. I'm not a biomedical engineer, and I don't pretend to be one. I'm the person between the clinic and the supplier, and my job is to make sure the equipment arrives with the right support around it.

Since 2021 I've processed maybe 185 orders—probably closer to 200, I'd have to check the system. The machines vary, but the buying failures don't: manuals stored behind dealer logins, error codes spread across different PDFs, training that sounded like it was included but wasn't. Here are the seven checks I run before approving any capital medical device purchase. No theory, no long background.

The 7-Step Equipment Procurement Checklist

Step 1: Name the operator and the setting before you compare prices

Before looking at vendors, write down who will actually use the equipment. A patient nebulizer is not the same as a staff-only ICU monitor. A wheelchair used by a child is not the same as one used by a bariatric adult. Every difference changes the training materials, the manual, and the service coverage. The obvious operator might not be the person reading the screen; some devices are operated by caregivers after a short training session. Note that in the purchase request from day one.

If the order says nebulizer, add model-specific patient instructions on the same line. On one occasion, we ordered a compressor nebulizer without specifying who teaches the patient. The supplier sent a packet with sales material and expected staff to know the rest. Asking for a model-specific how-to-use-a-nebulizer document before signing is much easier than chasing it later.

Step 2: Get the error-code document as a separate deliverable

A manual is not enough. If you buy electric wheelchairs, you also need the error-code guide. When a chair stops moving, the display may show a code, and the service tech will ask whether you have the current list. I now request a Permobil M3 error codes PDF for every Permobil M3 chair we buy. That doc goes in our local equipment folder, not on a dealer portal.

The reason is simple: without error-code definitions, an intermittent warning can turn into a full service call. With the code guide, our technicians can do the safe troubleshooting steps and then escalate if needed. If a vendor says the error codes are proprietary, ask whether an OEM-approved copy can be issued to your biomedical team. Make it a contract line item.

Step 3: Verify manual version, language, and transferability

Manuals are not all current by default. Ask the vendor to confirm that the PDF or printed manual matches the model and software level you are buying. For a Permobil C300, I ask directly for the current Permobil C300 manual, with publication date and version number. I also ask if the manual is allowed to be distributed internally. A restricted dealer portal is not a manual when the dealer relationship changes.

Under FDA device labeling requirements, most medical devices must have adequate directions for use or an applicable exemption. For practical purposes, the supplier should treat the instruction manual as part of the device, not as a sales extra. If they tell you no manual is available, put that in writing and ask for the regulatory basis. It's a useful red flag.

Step 4: Make training a numbered line item

Training is a deliverable. The line includes training is almost worthless because the vendor can check it with a fifteen-minute demo. Instead, ask exactly how many people will be trained, for how long, in person or remote, and whether training covers the specific user population.

For a nebulizer, the question that often exposes gaps is: walk me through how you would teach a first-time caregiver to use this model. A lot of vendors can demo the device, but fewer can explain cleaning, maintenance, and medicine cup assembly in plain language. If your clinic runs respiratory support, you should be able to produce a manufacturer's patient guidance document before the first order is filled. That document should be model-specific.

For an ICU monitor, training usually has another layer: clinical alarm settings and backup. That's not something I invent. We ask the clinical engineering team to review the install checklist while the representative is still on-site.

Step 5: Create the equipment folder before installation day

I get a folder started when the PO is sent, not when the device arrives. It holds the quote, current manual version, error-code reference, serial number, firmware revision, and service contact list.

For a hematology analyzer, the folder should also include the pre-installation electrical and space requirements, the calibration certificate, and the daily QC schedule. I learned that in 2022 when our lab analyzer arrived before the right power outlet was ready. That wasn't a supplier problem; it was a coordination problem, and the folder was the fix.

For an ICU monitor, include the network setup sheet and the manufacturer's default configuration list if one exists. One person can collect that material; another person changes jobs, and the next person doesn't have to start from zero.

Step 6: Buy response time as if you'll need it

If every device worked perfectly, choosing the cheapest service option would be fine. The problem is that nobody knows which device will fail. Service response time is a decision I make while I can still think clearly, not during a Friday afternoon breakdown.

I used to choose the standard service plan that said response within one week. We were told it was fine. When a patient monitor went down before a weekend, I learned the difference between a response and a resolution. A vendor can respond by email within 24 hours and not fix anything for another week.

For the small number of devices that affect scheduled care, purchase certainty. In early 2024 I added about 12% to the service contract for an ICU monitor to get a guaranteed 48-hour loaner. That fee bought us a fixed backup date. The cheaper option saved roughly $900, but it had no date. If I can't tell the internal manager when the monitor will be replaced, I haven't bought service. I've bought uncertainty.

I also keep FTC advertising guidance in mind: service claims need to be truthful and substantiated. That's why I ask for a promised response time in the quote, not in a sales conversation.

Step 7: Plan the battery and consumables before the first service call

This step is listed last but should be considered during step one. If a powered wheelchair or portable ICU monitor has a battery pack, ask for the part number, expected shipping time, and replacement instructions. Some replacements are safe for facility staff to do; some are manufacturer-service only. Find out now.

For a hematology analyzer, consumables may be in the quote, but not all of them are. Reagents, cuvettes, controls, and tubing need to be itemized. If you run a nebulizer program, masks, tubing, and filters matter. This isn't the glamorous part. It's the part that prevents we can't use the device because we're waiting for a filter emails.

Common Mistakes I Still See

Accepting the manual is online without verifying the current version. An online library is not always up to date, and URLs from different suppliers create documentation orphans.

Assuming the service contract includes a loaner. It often doesn't. Ask whether loaner appears in the written agreement.

Relying on verbal response time. If the sales rep says 24-hour support, put it in the purchase order and define it. A 24-hour response is not the same as a 24-hour repair.

Not checking distribution rights for the manual. I assumed our biomedical team could copy an OEM manual into our internal system. Turned out the PDF had restrictions. Ask permission to copy it for archival and training purposes, or request a manufacturer version without those limits.

This whole process sounds like additional work before you even receive the equipment. It is. But the alternative is work after the equipment fails, usually with a patient waiting. I still forget to ask a question, and I treat that as normal. The value of this checklist is that it makes the boring part visible. When I've followed it, procurement gets smoother. When I've skipped steps—especially manual version and service response—I've paid for it later.

I wrote this from my experience as of early 2025. Model numbers and support policies change, so use this as an outline. Your equipment folder is the source of truth for your current devices.
