This article isn't about bashing budget vendors. It's about comparing two procurement philosophies: “Lowest initial price” vs. “Best total value”. So When Does “Cheap” Make Sense?

I'm not saying every cheap option is a trap. There are use cases where low upfront cost can be the value play:

  • Non-critical, disposable items (e.g., exam gloves, gauze)—if they meet basic specs, price matters.
  • Short-term rentals or pilot equipment you plan to replace.
  • Devices with very low technical complexity where failure doesn't cause harm or downtime (maybe a basic pulse oximeter? Even then, check accuracy).

But for anything that impacts patient safety, clinical workflow, or regulatory compliance, my rule of thumb is: Never let the purchase price be the deciding factor. Calculate the true total cost of ownership over 3–5 years, including maintenance, documentation, training, downtime, and risk.

If you're unsure, run a small pilot like we did with the dental units—but design the pilot to reveal hidden costs, not just initial functionality. Track service calls, nurse complaints, and calibration issues.

Bottom line: I've saved maybe $200k in paper savings over my career, and spent $300k fixing them. I'd rather skip the cheap path and invest in things that work from day one. Your patients (and your CFO) will thank you—eventually.
